Soul Care Clinical Services
By scheduling an appointment, you agree to the following terms:
1. Physician Order Requirement
A valid physician or authorized provider order is required prior to specimen collection. Orders must be uploaded during booking or provided before the appointment. Services cannot be performed without proper documentation.
2. Scope of Services
Soul Care Clinical Services provides mobile specimen collection only.
We do not diagnose, interpret laboratory results, prescribe treatment, or provide medical advice. All questions regarding testing and results must be directed to your ordering provider.
3. Payment Policy
Payment is due at the time of booking unless prior arrangements are made.
Service fees cover specimen collection and mobile convenience only.
Laboratory testing fees are separate and billed by the laboratory.
All completed services are non‑refundable.
4. Insurance Disclaimer
Soul Care Clinical Services does not bill insurance.
If specimens are delivered to Quest Diagnostics or Labcorp, insurance information may be collected for laboratory billing purposes only. Clients are responsible for verifying their laboratory coverage and benefits.
5. Cancellation & Rescheduling Policy
Appointments must be canceled or rescheduled at least 24 hours in advance.
Late cancellations may be subject to a fee.
Same‑day cancellations or no‑shows may result in forfeiture of payment.
Emergency situations will be reviewed on a case‑by‑case basis.
6. No‑Show Policy
If the client is not present at the confirmed time and location, the appointment will be considered a no‑show and fees may apply.
Liability & Risk Acknowledgment
7. Assumption of Risk
Venipuncture and specimen collection involve minor but known risks, including but not limited to:
Bruising
Bleeding
Dizziness
Fainting
Infection
Discomfort
By booking services, the client acknowledges and accepts these inherent risks.
8. Limitation of Liability
Soul Care Clinical Services shall not be held liable for:
Laboratory errors
Delays in test results
Insurance claim denials
Provider order inaccuracies
Complications resulting from undisclosed medical conditions
Damages arising from circumstances beyond our control
Liability is limited to the amount paid for the service provided.
9. Release of Claims
Clients agree to release and hold harmless Soul Care Clinical Services, its owner, employees, contractors, and representatives from any claims, damages, or liabilities arising from services rendered, except in cases of proven gross negligence or willful misconduct.
10. Safe Environment Requirement
Clients must provide a safe, clean environment for services.
Soul Care Clinical Services reserves the right to refuse or discontinue service if the environment is deemed unsafe, unsanitary, or inappropriate.
11. Client Responsibilities
Clients are responsible for:
Providing accurate personal information
Providing correct address and access instructions
Securing pets
Following fasting or preparation instructions
Disclosing relevant medical conditions (e.g., bleeding disorders, fainting history)
12. Specimen Handling Disclaimer
Once a specimen is transferred to the laboratory, Soul Care Clinical Services is not responsible for processing, testing, storage, or result reporting.
13. Force Majeure
Soul Care Clinical Services is not liable for delays or inability to perform services due to events beyond reasonable control, including severe weather, emergencies, traffic disruptions, or public health situations.
14. Privacy & Confidentiality
All personal health information is handled in accordance with applicable privacy laws. Client information is used solely for service coordination and laboratory submission purposes.
15. Right to Refuse Service
Soul Care Clinical Services reserves the right to refuse service for:
Lack of proper documentation
Non‑payment
Unsafe conditions
Inappropriate behavior
